Insurance & Billing
Our eyecare providers accept most major insurance carriers. Please check with your insurance carrier to verify that your insurance will be accepted at the Eye Institute. We will file your insurance claim on your behalf as a courtesy. If you are covered by a plan with which our eyecare providers do not participate, payment is expected at time of service.
Payment for office services, contact lenses, and glasses, which may include co-pays, co-insurance, and/or deductibles, are expected at the time of service.
If you are unable to pay for your services, we will be happy to reschedule your appointment.
We are here to service the community, and because of our commitment to caring for the people in our community we do have programs to assist the public in times of financial hardship. Please contact the clinic at 623-537-6000 and ask for the Manager of Patient Accounts regarding the sliding fee scale and to learn more about the MWU Cares Program.
